From 1 November 2025, the Home Care Packages Program became part of the new Support at Home framework.
Our Home Support Program began delivering these services under Support at Home, helping people stay safe, well and independent at home.
Home Support Program at Central Highlands Rural Health
As you get older, you or a loved one might need a little more support day-to-day. Our government-funded Home Support Program is designed to help you live safely and comfortably at home, and keep doing the things that make you happy.
Maybe you need a hand with the shopping, running errands, or personal care. Or perhaps some help with meals, cleaning, or getting out and about. Whatever support looks like for you, we’re here to make things easier and more manageable.
At Central Highlands Rural Health, you’ll know our names and we’ll know yours. Many of our staff live and work locally, and we’re proud to support people across our region to stay independent at home.
Living independently at home can become harder as you get older. Getting a bit of help with everyday tasks can make a big difference – allowing you to stay in your home for longer and enjoy a better quality of life. Help at home looks different for everyone.
For some, it’s help with shopping or cooking. For others, it’s personal care, transport, or nursing support.
Whatever your needs, we’ll work with you to design a plan that fits your goals and lifestyle.
Once you’re approved for the Support at Home program, we’ll talk with you about the kinds of services you’d like. Together, we’ll design a personalised care plan and agreement that meets your needs and preferences.
If you’re an older person who needs some help to stay independent at home, you may be eligible for government-funded Support at Home services. Eligibility is based on your age, needs, and level of support required—not your income or where you live.
You may be eligible if you:
A member of the Aged Care Assessment Team (ACAT) will work with you to understand your needs and determine what level of support is right for you.

The cost of your Support at Home services will depend on the type of support you need and how often you receive it.
If you’re eligible for government funding, your fees will be subsidised under the Support at Home program.
We’ll work with you to create a personalised plan and give you a clear breakdown of any costs before you start services.
