The Post-Acute Care (PAC) program aims to facilitate a safe and timely discharge from hospital. The person returns home with an appropriate package of community-based support. They are also linked with long-term support in the community if required.
If you have been discharged from a public hospital (including emergency departments, acute and sub-acute services) with limited support at home, you can be referred to PAC. You will need to be assessed as requiring short term, community-based help to assist in your recuperation, and to ensure a safe and timely discharge.
If you had another program providing support prior to the hospital admission, it is expected that these services will resume post discharge.
Services offered by PAC are:
− Community Nursing
− Personal Care
− Home Help
− Shopping assistance
The care can be made available for up to four weeks after you leave hospital (if you don’t return as a patient in that time). The program is designed to help you to get back to independent living and confidently looking after yourself.
