The Post-Acute Care (PAC) program aims to facilitate a safe and timely discharge from hospital. The person returns home with an appropriate package of community-based support. They are also linked with long-term support in the community if required.
If you have been discharged from a public hospital (including emergency departments, acute and sub-acute services) with limited support at home, you can be referred to PAC. You will need to be assessed as requiring short-term, community-based help to assist in your recuperation, and to ensure a safe and timely discharge.
If you had another program providing support prior to the hospital admission, it is expected that these services will resume post-discharge.

The care can be made available for up to four weeks after you leave the hospital (if you don’t return as a patient in that time). The program is designed to help you to get back to independent living and confidently look after yourself.
