In addition to quality hospital facilities, the Central Highlands Community Nursing team gets on the road right across the Macedon Ranges and Hepburn Shires, to deliver thousands of home visits each year.
Our friendly professionals deliver expertise, equipment and empathy to support you to remain independent in your own home.
Because we’re local hospitals and a healthcare hubs we understand the transition between hospital and home, and we make it easier for you to move between them as you need to – including contact with the larger tertiary hospitals.
Our highly skilled team is multi-disciplinary and includes qualified nurses, occupational therapists and physios, plus 24/7 access to urgent care with GPs and specialist nurses at the hospital.

It can be confusing to understand what care program you need, but we can help you to work out what you’re eligible for, so that you can access the program that best meets your needs, whether it’s private or through NDIS or My Aged Care.
Our network means we can help you to navigate the healthcare system to connect with the right services, resources and professionals.
As you get older or if you become unwell you might need a hand to manage your day-to-day health, like diabetes, wounds or chronic pain management. Maybe you just need a little support as you recover from a hospital stay.
Our Community Nursing teams can help you to maintain your independence, and to stay at home and in your community for longer.
Experienced nurses are available for people of all ages, to help treat or manage your health. You might need assistance dressing a wound, looking after a catheter or managing medication.
We make it easy for you to access quality support without having to travel or rely on the availability of friends or family. We’ll get on the road to visit and care for you at home. You can be reassured that you’re in the hands of qualified professionals, who are well trained and connected as part of your local hospital.
If you are over the age of 65 and frail you may be eligible for nursing and specialised support through the Australian Government Department of Health under the Commonwealth Home Support Program (CHSP). To find out if you are eligible contact My Aged Care on 1800 200 422 and ask for an assessment.
If you are under the age of 65, you may be eligible for nursing through the Victorian State Government Department of Health Home and Community Care Program for Younger People (HACCPYP).
Central Highlands Rural Health is also the provider of Community Nursing for the Department of Veteran Affairs. To find out if you are eligible for these subsidised services, talk to the Coordinator on (03) 5321 6570 for Hepburn Shire and (03) 5422 9911 for Kyneton and the Macedon Ranges.
Fees may apply and the cost is dependent upon your eligibility for a subsidy. The Coordinator will talk with you about any fees you may be required to pay and what is included in the cost.
