Our service provides information, education and clinical care to people living with diabetes and/or their carers and families within the community.
We provide care for people with all types of diabetes across the lifespan.
This includes education about prevention and management of diabetes, as well as supporting our community to make healthy lifestyle choices, to ensure optimal health for all.
At Central Highlands Rural Health we work together with your other health care team members to individualise your diabetes needs.
Current Service Provision: Community Health, Acute In-patients, Residential Aged Care, CHSP, TCP.
Referrals: processed via ACCESS.
We are located at Clunes, Creswick, Daylesford and Trentham from Monday-Thursday.
You can call one of our friendly staff as a self-referral, or if you prefer, you can see your GP who can then send a referral to the Diabetes Education Service.
