Central Highlands Rural Health has achieved Victorian-first recognition for excellence in Aged Care by a global healthcare benchmarking institute. We recently received special recognition as an Age-Friendly Health Systems — Committed to Care Excellence by the Institute for Healthcare Improvement.
As an active participant in the global Age-Friendly Health Systems movement, we join the ranks of organisations achieving success toward excellence in care for older adults.
This recognition requires working toward the reliable practice of a set of four evidence-based elements of high-quality care for all older adults, known as the 4Ms: What Matters, Medication, Mind, and Mobility.
Quote from Susan Jennings, Executive Director Clinical Operations, Chief Nursing Officer, DON Kyneton:
“The Age-Friendly Health Systems initiative continues to be an important part of our overarching vision to provide every older adult in our community with the best care possible. Asking the patient about what matters to them makes them feel seen, heard and known and sets the stage for a relationship of mutual trust and respect and better health outcomes.“
Earlier this year, our organisation was a finalist in the Victorian Public Healthcare Awards in the Excellence in Aged Care category for implementing the 4Ms evidence-based practices changes across our three Residential Aged Care facilities.
